Job Summary:
Provides customer service, evaluates applicants and determines eligibility, provides counseling, maintains records, provides training, provides program information at meetings, maintains inventory and provides data entry.
Job Duties:
Routine duties shall include providing health care services to individuals eligible for services in accordance with the self-governance compact and funding agreement between the Cherokee Nation and the United States executed under the authority of the Indian Self-Determination and Education Assistance Act. Provides courteous and respectful service to the public. Determines program eligibility by evaluating applicant's dietary habits and medical and income assessments. Informs program applicants of their rights and responsibilities. Obtains height and weight information.
Provides nutrition and health counseling to clients based on their health risk factors and individual needs. Promotes breastfeeding by providing education and counseling. Maintains records, logs, and documentation of individual participant interactions and findings. Maintains Case-loads Management according to program standards. Provides appropriate referrals to applicants/participants.
Provides training to staff. Provides support in the area of quality assurance according to established standards. Provides program information and acquires knowledge for referrals at community organization meetings and activities. Maintains inventory of assigned program equipment and supplies. Performs data entry in an accurate and precise manner. Other duties may be assigned.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.

Qualifications:
EDUCATION REQUIREMENT
Bachelor's degree from a four-year college or university in Nutritional Sciences, Community Nutrition, Clinical Nutrition, Dietetics, Public Health Nutrition, Social Services, Early Childhood Development, Home Economics, Teaching, or a related field of study; or six years related experience; or equivalent combination of education and experience.
CERTIFICATES, LICENSES, REGISTRATIONS
Must possess a valid driver's license with a driving history verified through a motor vehicle report that meets requirements for Cherokee Nation underwriting rating.
OTHER SKILLS AND ABILITIES
Must have knowledge of and the ability to operate standard computer programs.
OTHER QUALIFICATIONS
Must meet and maintain pre-employment and periodic background investigation and adjudication for child care.
Employee must not be and will not be under sanction by the United States Department of Health and Human Services Office of the Inspector General (OIG) or by the General Services Administration (GSA) or listed on the OIGs Cumulative Sanction Report or the GSAs List of Excluded Providers or listed on the OIGs List of Excluded Individuals/Entities (LEIE).
